The largest nerve in the body, the sciatic nerve is about an inch wide and is comprised of multiple spinal nerves.
Sciatica arises when this nerve is being pinched – either by a herniated disc or a bone spur – in the area where the nerve exits the spinal column in the lower back.
Sciatica is generally caused by a pressure or irritation on the Sciatic nerve or on one the nerve roots that exit the lumbar spine and form the sciatic nerve. This pressure can be caused in general by two ways and at two different points in the body.
The least common is when the piriformis muscle (which the sciatic nerve runs through in about 15% of the population) actually clamps down or puts pressure on the sciatic nerve. Very similar to stepping on a garden hose when the water is on, the nerve signals are no longer able to travel down the sciatic nerve and shooting pain, numbness or tingling can occur.
The most common cause of sciatica is when one, or several of the five lumbar nerve roots are compressed or irritated. These nerve roots exit the spine through holes called intervertebral foramen. This size of this hole or foramen can be greatly reduced when vertebrae become out of place or misaligned, when the spine develops arthritis and degeneration, or, when discs of the spine swell due to inflammation, protrusion or herniation.
These last three are usually a result of vertebrae being out of alignment for an extended period of time causing degeneration and are the Most Common Cause of Sciatica!!
